It is different from a lost title situation where the paperwork was never in your name at all; for a duplicate, the state simply reissues what it already has on record.

The core form is HSMV 82101, the Application for Duplicate or Corrected Title. Sounds simple - but applications get rejected for small mistakes: a missing signature, the wrong lienholder listed, an address that does not match the record, or an unpaid lien that nobody told you about.
